The ultimate aim of the transition process is to ensure optimal care for the individual from adolescence onward, setting them up for excellent health outcomes throughout the rest of their lifetime.

For many patients transitioning to adulthood with congenital urologic issues, the primary urologic management goals focus on:

  • Maintaining function of the upper urinary tracts

  • Achieving continence

  • Optimizing overall quality of life

  • Minimizing urinary tract infections

  • Improving sexual function

  • Maximizing fertility potential
